Agreement for Use of Hearing Officer Listings

CSHA does not recommend or make referrals of hearing officer candidates to any individuals or entities. CSHA does not warrant or verify the qualifications or experience of any person appearing on the CSHA Hearing Officer List (“List”). Inclusion of a person on the List does not imply a referral, endorsement, or recommendation by CSHA, nor does the omission of any individual indicate CSHA disapproval of any persons. Prior to engaging a hearing officer, each individual reviewing the List (“viewer”) should perform his or her own due diligence in determining the qualifications or suitability of any hearing officer candidate that the viewer proposes to engage or recommend, whether on his or her own behalf or on behalf of another individual or entity (“represented entity”). By accessing the List, each viewer and his or her represented entity is deemed to (a) acknowledge the foregoing; (b) waive any claim, contention or liability against CSHA, its directors and members of its governing boards and its officers, employees, agents, attorneys, consultants and representatives that arises from the use of the List or any particular hearing officer; and (c) to the fullest extent permitted by applicable law, indemnify, hold harmless and release CSHA, its directors and members of its governing boards and its officers, employees, agents, attorneys, consultants and representatives from any and all liability to that viewer, to a person or entity claiming through the viewer, and to his or her represented entity with respect to use of the List. It is the viewer’s responsibility to ensure that any represented entity is aware of the provisions in this paragraph; however, the viewer’s failure to do so does not invalidate any of the provisions in this paragraph.

In order to participate in the program, you must be a member of CSHA, meet the requisite qualifications and make attestations for either the “Completed Hearing” or “General List.”

Remittance to CSHA of an operating fee of $50 per year and a signed Attestation Form is also required. In addition, eligible participants must furnish to CSHA a resume containing at a minimum the following information, in addition to your name and contact information:
